This procedure allows you to obtain Polish civil status documents based on foreign records of birth, marriage, divorce, or death. We work with the following documents:

Birth certificate;
Marriage certificate;
Death certificate;
Divorce certificate.

What’s Included:

The price includes 2 translations (Russian, Ukrainian, or Belarusian only).
We register your certificates in Warsaw under a power of attorney.
The original certificate is retained in the archives. A Polish birth certificate (akt urodzenia) or marriage certificate (akt małżeństwa) is issued in its place.
Once the records have been entered into the Polish civil registry, you can obtain copies of your Polish birth or marriage certificate as often as needed from a Civil Registry Office (Urząd Stanu Cywilnego).
